I accidentally clicked the X button on my window, and it closed the game. when i did that, i was fighting Cladt and had just beat him. When i returned, Akia was gone. But what got me thinking is that after returning to the village, she had the star as if I had never done the quest within the last ten minutes.

It's easy to explain why it happened (signing out makes all NPCs directly linked to despawn, which in case of Akia makes your quest fail).
I'll have to mark it so the quest no longer fails after you killed Cladt. Thanks for letting me know.
