Regular hits is one of the possible outcomes of attacks and abilities, along with critical strikes and misses.
An attack or a spell that is not a critical strike and does not miss is a regular hit. Everything else being equal, players have a 0% chance to miss, and a 5% chance to land a critical strike. This is offset by various stats, such as levels, dexterity and agility.
Most direct damage causes your character to flinch, interrupting movement and delaying spell casts for the fraction of a second.
